Diver Liability Release

This form must be filled out and signed if you are diving with Sea the World Scuba Center Inc & Staff. Please print name, sign and date. Bring the completed, signed form with you to any Sea the World Scuba Center Inc's diving function or email back to us prior. Send to support@SeatheWorld.US. Students under the age of 18 need a signature from a parent or guardian. The form is located below.

Scuba Medical Questionnaire with Doctor's Sign-Off Form

Medical Questionnaires are required before any recreational scuba activity. If any of these items apply to you, we must request that you consult with a physician prior to participating in scuba diving. Please print the entire document to share with your physician. Sign and fill out the questionnaire on page one with the complete words, "YES" and "NO." Your doctor should complete page two. Bring the completed, signed form with you to class. Students under the age of 18 need a signature from a parent or guardian.
